Firstly I started out with rubber rings and after my nut sack was stretched out some I graduated to the metal rings.
Having a loose scrotum is key. And little lubrication is essential.
To answer your question, yes my balls do swell when aroused and getting them off at that moment would be difficult.
Time is your friend, once things subside they pop out without much resistance.
Sometimes I wear them for days or weeks, just not loaded up to the hilt.
But I do have a pair of bolt cutter handy if something unusual were to happen ;>)
